Our Referral Form is a useful tool created to connect community members to our partner agencies for Intervention and Prevention services. Upon successful completion of the form, the agency representative that serves the area of the prospective participant will be in contact to provide further information.
In order for youth to be eligible for GRYD Prevention services, referrals to the program must meet the following criteria:
Youth must be between ages 10 and 15 years old; and
Exhibit 2 or more of the following risk factors: 1. Antisocial Tendencies 2. Weak Parental Supervision 3. Critical Life Events 4. Impulsive Risk Taking 5. Guilt Neutralization 6. Negative Peer Influence 7. Peer Delinquency 8. Family Gang Influence 9. Self-Reported Delinquency
In order to be eligible for GRYD Intervention services, referrals to the program must meet the following criteria:
Referrals must be between the ages of 14 – 25;
Be a tagger or member/affiliate of a gang or crew.
